About These Terms
This page outlines the agreement between A Healing Heart Medical Clinic and our patients. These policies ensure medical safety, protect patient privacy under the PDPA, and maintain a high standard of clinical excellence. By booking an appointment or utilizing our services, you acknowledge and agree to the practices described below.
General Clinic Policies
- Punctuality: Please arrive 5–10 minutes before your scheduled slot. Late arrivals may be rescheduled to ensure other patients are seen on time.
- Cancellations: Please provide at least 24-hour notice for cancellations. Frequent no-shows may require pre-payment for future bookings.
- Clinical Priority: Medical emergencies or complex cardiac cases may take precedence over scheduled times. We appreciate your patience.
- Subsidies: CHAS and MediSave eligibility is verified via Singpass. Original NRIC/FIN must be presented at registration.
- Payments: All fees (consultation, medication, and procedures) are due at the point of service. We accept NETS, PayNow, Cash, and Credit Cards.
- Billing Disputes: We practice transparent billing. Patients are encouraged to review their itemized charges at the front desk before making payment. Retrospective claims of "overcharging" will not be entertained once the bill is settled on-site.
- Consultation Fees: Once a consultation has commenced, the fee is earned and strictly non-refundable, regardless of the patient's agreement with the clinical outcome or diagnosis.
- Pre-paid Diagnostic Scans & Tests: Specialized scans (e.g., CT, MRI, or specialized blood panels) involve stock allocation and external coordination.
- Refunds will not be issued once the lab request is triggered or the imaging slot is locked.
- If a patient chooses not to proceed after payment but before the request is triggered, a mandatory Administrative Fee applies.
- Wrong Card / Payment Errors: It is the patient's responsibility to ensure the correct payment method is used. If a refund is requested to switch credit cards after a transaction is processed, an Administrative Fee will be deducted to cover merchant surcharges and bank costs.
- Medications & Vaccinations: To maintain healthcare safety and cold-chain integrity, these items cannot be returned or refunded once they leave the clinic or are prepared/administered.
- Refund Timeline: Approved refunds are not immediate.
- Clinic processing takes 3–5 working days.
- Actual credit to your account is subject to Bank Processing Times (typically 7–14 working days). The clinic is not responsible for bank-imposed delays.
- Late Cancellation: We reserve the right to forfeit deposits for specialized appointments canceled with less than 24 hours' notice.
In accordance with the Personal Data Protection Act (PDPA) and SMC Ethical Guidelines, we collect and manage your data solely for clinical care and regulatory compliance.
- NEHR: As part of Singapore's national healthcare framework, your medical information will be uploaded to the National Electronic Health Record (NEHR) to ensure seamless continuity of care across the healthcare ecosystem.
- Referral Continuity of Care: If you were referred to us by another healthcare provider (GP, Medical Centre, Specialist, or Hospital) for investigations (e.g., Blood Tests, ECG, Scans) or medical review, we are professionally and ethically obliged to share your results and clinical findings back with the referring practitioner. By proceeding with the investigation, you acknowledge and consent to this clinical data exchange.
- Third-Party Disclosure: We will not disclose your medical information to non-medical third parties (e.g., employers, insurance agents) without your explicit written authorization, except where required by law or for infectious disease reporting.
- Communication: You consent to receive appointment reminders, clinical follow-up notifications, and lab result alerts via SMS, Voice Call, or WhatsApp.
Patient Declaration & Authorization
In accordance with SMC Ethical Code and PDPA Guidelines
Authorization for Release of Medical Information:
- Personal Consent: E.g. [Name and Identity Card Number] authorize A Healing Heart Medical Clinic to release my clinical findings and history to the designated recipient.
- Third-Party & Complexity Fees: E.g. [Name, Identity Card Number, Name of Organisation] acknowledge that fees for medical reports are determined by the extent of review and clinical complexity.
- Standard fees apply to simple abstracts; higher professional fees apply to reports requiring extensive chronological review or interpretation of complex cardiac imaging (CT/MRI/ECG).
- Third-party requests (Insurance/Legal) incur mandatory professional fees which must be settled before the report is released.
- Letter of Authorisation (LOA): Third-party requests must be accompanied by a formal LOA with company letterhead, patient signature, and ID number.
- Minors/Incapacitated Patients: Authorization must be granted by a parent, legal guardian, or appointed representative (supported by Birth Cert, Grant of Probate, or LPA).
- Accuracy & Factual Integrity: I understand that reports are factual representations based on medical records and cannot be altered to suit personal preference or claim outcomes.
- Non-Refundable: Fees for medical reports are strictly non-refundable once the doctor has commenced the clinical review.
- Consent Validity: Please download and fill up the application form below. This authorization is valid for six (6) months from the date of signature.

Note on Documentation: Verification of identity (NRIC/FIN/Passport) is mandatory. Processing takes 7 to 14 working days after full payment and complete documentation are received.
Medical Emergencies
A Healing Heart Medical Clinic is an intermediate care facility and not an A&E department.
